Why do we have such issues with wides

It would indicate lack of control caused by a loose rather than compact action and/or head movement in delivery stride, which is normally down to 3 things:

Fatigue
Poor coaching
Poor discipline

I'd say that it's probably poor or insufficient coaching.

I don't agree. His best weapon is back of a length, 4th stump, or a bit shorter straight at the guy's sternum, particularly if the pitch is holding.
